• Stamped on company letterhead and signed by the authorized signatory visa request petition addressed to the consulate general
• 2 Application Forms to be filled

Chad
Chad (French: Tchad; Arabic: تشاد) is a landlocked country in Central Africa.
It is bordered by Libya to the north, Niger, Nigeria and Cameroon to the west, the Central African Republic to the south, and Sudan to the east. Most of the country is covered by the Sahara Desert. The country takes its name from Lake Chad, which was once Africa's second largest lake, but has now shrunk to 10% of its normal size.
Its highest peak is Mount Emi Koussi, and its largest city is N'Djamena, which is also the capital. Chad is a religiously homogeneous country. According to a study conducted by the Pew Research Center in 2010, 57.7% of the country's population is Muslim, 21.5% is Catholic, and 16.6% is Protestant.[6][7]
